Welcome to Ottawa Township High School:

                   In order to register at Ottawa Township High School, you must obtain key information from the school you are leaving.
                   The information you need to request is as follows:

1) a copy of your high school transcript

2) a copy of your 9th grade physical, complete with all your immunization records

3) a student transfer form completed by the school you are leaving (a blank copy of this form is enclosed in this packet if you need to present your former school a copy)

 4) a copy of your last report card

 5) proof of residency in Ottawa High School's district (this can be a paid utility bill, copy of a mortgage or rent receipt, etc.)

 6) guardianship document (if appropriate)

                After you have the above information in your possession, call O.T.H.S. (815-433-1323) to make a registration appointment with the correct
                class counselor. Students under the age of 18 must have a parent/legal guardian with them at their registration appointment.

Summer hours that counselors are available for appointments are as follows:

              August 4 (Monday) - 9:00 to 12:00

                                      August 5 (Tuesday) - 9:00 to 12:00

                                      August 6 (Wednesday) - 9:00 to 12:00

                                      August 7 (Thursday) - 9:00 to 12:00

                                      August 11 (Monday) – 8:00 to 3:00 (9^th & 10^th registration completion)

                                      August 12 (Tuesday) - 8:00 to 3:00 (11 & 12^th registration completion)

                                      August 13 (Wednesday) - 9:00 to 12:00

                                     August 14 (Thursday) - 9:00 to 12:00

                                     August 15 (Friday) - 9:00 to 12:00

 

           Completing the enclosed enrollment information sheet will expedite the registration process.
